By booking or purchasing services from Integrity Lab Solutions, you agree to the following terms:
Integrity Lab Solutions provides mobile specimen collection and transport services only. All laboratory testing is performed by certified third-party laboratories. We do not provide medical advice, diagnosis, or treatment.
Payment is required at the time of booking unless otherwise stated. All sales are final once services are rendered. Pricing reflects collection services and does not include laboratory testing fees unless specified.
Appointments are subject to availability. Clients must provide accurate information, valid lab orders when required, and follow any preparation instructions. Failure to do so may result in rescheduling.
A minimum of 24 hours’ notice is required for cancellations or rescheduling. Missed appointments or late cancellations may be subject to a fee.
Integrity Lab Solutions is not responsible for delays caused by shipping carriers, laboratory processing times, or incomplete documentation.
By using our services, you agree that Integrity Lab Solutions is not liable for any damages, losses, or outcomes related to laboratory testing or medical results.
We reserve the right to refuse or discontinue service if conditions are unsafe or service requirements are not met.
