BUSINESS POLICIES:

 

Integrity Lab Solutions

Integrity Lab Solutions provides professional mobile and in-office specimen collection services throughout New Jersey. All laboratory testing is performed by certified third-party laboratories. Integrity Lab Solutions does not diagnose, treat, interpret laboratory results, or provide medical advice.

 

Appointments & Scheduling:

Appointments are available by reservation only and are subject to availability.

Services may be performed at:

• Client homes
• Offices and workplaces
• Assisted living facilities
• Healthcare facilities
• Our designated office location

Clients are responsible for providing accurate contact information, service location information, and any required documentation before their appointment.

 

Payment Policy:

Payment is required at the time of booking unless otherwise stated.

Collection fees cover appointment scheduling, specimen collection services, administrative processing, supplies, and travel when applicable.

Laboratory testing fees are separate unless specifically stated.

 

Mobile Travel Fees:

Travel fees are based on distance from our service location:

• 0–10 miles: Included
• 10–20 miles: $35
• 20–30 miles: $60

Additional travel fees may apply outside our standard service area.

Travel fees are non-refundable once travel has begun.

 

Additional Patient Fee:

An additional fee may apply for each additional patient seen during the same appointment at the same location.

 

Cancellation Policy:

A minimum of 24 hours' notice is required to cancel or reschedule an appointment.

Appointments canceled with less than 24 hours' notice are not eligible for a refund.

 

No-Show Policy:

 

Mobile Appointments:

Clients must be available and reachable at the scheduled appointment time.

If Integrity Lab Solutions arrives at the appointment location and is unable to reach the client within 15 minutes of arrival, the appointment will be considered a no-show.

 

In-Office Appointments:

Clients arriving more than 15 minutes late may be required to reschedule based on availability.

Failure to arrive for a scheduled appointment will be considered a no-show.

 

No-show appointments are not eligible for a refund.

 

Rescheduling Policy:

One courtesy reschedule may be offered per appointment at the discretion of Integrity Lab Solutions.

 

Additional rescheduling requests may require a new appointment and payment.

If a mobile appointment must be rescheduled due to client unavailability, a new travel fee will apply to the return visit.

 

Refund Policy:

Refunds are not provided for

• Completed services
• No-show appointments
• Appointments canceled with less than 24 hours' notice
• Client refusal of service after arrival
• Collection attempts that cannot be completed due to client-related circumstances

 

Approved administrative refunds will be reviewed on a case-by-case basis and processed after the scheduled service date.

Any payment processing fees charged by third-party payment processors may be deducted from approved refunds.

 

Specimen Collection Policy:

Payment covers the scheduled appointment, collection attempt, travel time (if applicable), and supplies.

 

If a specimen cannot be collected due to:

• Difficult veins
• Dehydration
• Client refusal
• Client non-compliance
• Inability to safely perform the collection
• Other circumstances beyond the collector's control

the appointment fee remains non-refundable.

 

Please refer to our Redraw & Return Visit Policy for additional information regarding redraws and repeat collections.

 

Specialty Laboratory Kits:

Integrity Lab Solutions provides specimen collection and specimen transport services only.

Replacement kits, replacement shipping materials, or laboratory-specific redraw requirements may need to be coordinated directly through the ordering provider or laboratory.

Laboratory testing fees, kit fees, and laboratory refund policies are determined by the laboratory provider.

 

Patient Comfort & Safety:

Patients may request to pause or discontinue specimen collection at any time during the appointment.

Integrity Lab Solutions reserves the right to discontinue a collection if safety concerns arise or if the collection cannot be performed safely.

 

Privacy & Compliance:

Integrity Lab Solutions is committed to protecting client privacy. Personal and health information is handled in accordance with applicable privacy and HIPAA requirements.

 

Right to Refuse Service:

Integrity Lab Solutions reserves the right to refuse or discontinue service when:

• Required documentation is not provided
• The environment is unsafe
• The client is disruptive or abusive
• Service requirements cannot be met
• Safety or legal concerns exist

 

By booking an appointment with Integrity Lab Solutions, you acknowledge and agree to these Business Policies.
